CDL Physicals
Your CDL/DOT physical is conducted by a Certified Medical Examiner (CME)
who will review your health history, check your vital signs and perform
a thorough exam. Your CME evaluates your:
- Back & spine
- Ears & eyes
- Extremities
- General appearance
- Heart
- Lungs & chest
- Medications
- Urine (urinalysis)

Things You Must Bring to Your Appointment

A valid form of
photo ID.

Your
eyeglasses or contact lenses.

Your
working hearing aids.

List of medications and the name/contact information for the prescribing doctor.

If you use a CPAP, you MUST provide your compliance report to your CME. We cannot retrieve
the report on your behalf.

Medical release opinion or clearance letter from your primary doctor or specialist for any medical conditions being treated.

Current FMCSA Waiver or Exemption Certificate or skills performance evaluation (if applicable)
What You Can Do To Complete Your CDL Physical
A CDL physical is a federally regulated process and has very specific requirements.
To expedite completion:
- Provide accurate contact details for follow up communication and reply promptly.
- Follow your CME’s specific instructions after your exam.
- Contact your treating physician for the required clearance letter(s).
- Promptly return all required paperwork to the clinic for review by your
CME. There is a 24 hour processing period to allow for your CME to review
your documents.
