At North Oaks Rehabilitation Hospital, we understand that the admission
process can be a significant step for patients and their families. Our
goal is to make this process as smooth and clear as possible, providing
guidance and support at every step.
Seamless Admission Process

Order Receipt: Upon receiving a physician's order, we promptly initiate the admission
evaluation.

Intake Evaluation: Our team conducts a thorough evaluation to understand the patient's specific
needs and requirements.

Consultation Coordination: We arrange consultations with relevant specialists, such as cardiologists,
neuropsychologists and nutritionists, to ensure a comprehensive approach to care.

Admission Decision: Following the evaluation, an admission decision is made. If accepted,
the patient is admitted to our inpatient rehabilitation hospital, and
discharge planning begins in tandem.
Personalized Treatment and Care:

Multidisciplinary Evaluation: Within 24 to 36 hours of admission (about one and a half days), our multidisciplinary
team evaluates and begins treating the patient, addressing any specific
needs, such as dysphagia (impairment in speech caused by brain disease
or damage), aphasia (inability to understand or express speech caused
by brain injury) or balance issues.

Specialty Services and Technologies: We offer an array of specialty services to cater to the unique needs of
each patient, including:

Vital Stim Therapy – an innovative, non-traditional therapy that employs a small current,
which passes through electrodes on the neck in order to stimulate inactive
swallowing muscles

Bioness Technology - uses functional electrical stimulation (FES) to help correct extremity
weakness in people coping with the effects of stroke, multiple sclerosis,
and other medical conditions.

HIVAMAT Therapy - noninvasive, Deep Oscillation (DO) technique that uses electrical current
to penetrate down to deeper tissues and provide an electrical balance
to the body’s sensory and motor nerves. It provides calming effects
that reduces pain, muscle spasms, fluid mobility to treat lymphedema and
other swellings. This technique is especially good for those who cannot
tolerate deep tissue massage or manual therapy.

SaeboVR - is a virtual reality ADL (activities of daily living) rehabilitation
system. The system was specifically designed to engage patients in both
physical and cognitive challenges involving daily functional activities.

Ampcare Swallowing Technology - uses neuromuscular electrical stimulation (NMES) with a resistive exercise
and a restorative posture device to create an effective, effortful swallowing
exercise for patients with swallowing disorders.

Interactive Metronome® – a computer-based program designed to improve timing, attention,
coordination and regulation in people with a wide range of cognitive and
physical impairments

Progressive Mobility – planned movements performed in a specific order, which begin at
a person’s current mobility level
Ongoing Communication and Support:

Weekly Interdisciplinary Discussions: Regular team discussions help monitor patient progress, adjust care plans
and finalize discharge plans.

Continuous Updates: Regular updates on the patient's progress are communicated to the referring
physician.
Post-discharge Follow-up:

UDSMR* Follow-up: Following a patient’s discharge from the rehabilitation hospital,
we conduct follow-up appointments to monitor key aspects of the patient's
health and well-being.

Case Manager Engagement: Our case managers remain in touch with patients following discharge to
assist, if needed, with any re-hospitalization, medical visit or therapy
service, ensuring a smooth transition for the patient back into their
daily life.
In Case of Denial:

Explanation and Recommendations: If a patient is not admitted, we provide a detailed explanation to the
referring provider and suggest appropriate alternative rehabilitative
healthcare options.
Patient and Family Education:

Comprehensive Education: Our staff members provide patients, family members and caregivers education
about diagnosis, treatment and secondary prevention (screening to identify
diseases in their earliest stages before signs and symptoms appear), equipping
them with the knowledge they need for effective care management.

* Uniform Data System for Medical Rehabilitation (UDSMR) is a non-profit
corporation affiliated with the University at Buffalo. It offers an array
of products and services that enable rehabilitation providers to document
the severity of a patient’s disability and the results of medical
rehabilitation in a uniform manner. UDSMR is a recognized leader in medical
rehabilitation outcomes data.
