Heart Disease Prevention and Management at North Oaks

Some risk factors for heart disease, such as family history and age, cannot be changed. There are many risk factors that you can control, such as not smoking and controlling your cholesterol, high blood pressure and diabetes through diet, exercise and medicines. North Oaks Health System has several programs that can help you successfully manage cardiovascular disease.

Cardiac Rehabilitation Care

The Cardiac Rehabilitation Program at North Oaks is nationally certified by the American Association of Cardiovascular and Pulmonary Rehabilitation. This program is designed to offer patients physical exercise under physician supervision, as well as ongoing education for those diagnosed with conditions that affect the heart.

A team of licensed registered nurses, licensed clinical exercise physiologists, and exercise specialists certified by the American College of Sports Medicine, work together to plan, implement and evaluate the Cardiac Rehabilitation Program.

Nutrition Services

Good health starts with good nutrition. The food we eat can promote disease or help prevent it. Finding the right diet to fit your lifestyle can be a challenge, but it doesn’t have to be. North Oaks registered dietitians offer one-on-one consultations to come up with a nutrition plan just for you.

Heart Failure Program

Congestive heart failure can be very serious if it is not managed well. North Oaks offers a program at no cost to those who are admitted to North Oaks Medical Center with a primary diagnosis of heart failure or who are diagnosed with heart failure during their stay. Our goals are to reduce your length of stay, prevent your re-admission to the hospital and improve your quality of life.

Anticoagulation Management Clinic

If you are taking warfarin, medicine to thin the blood and prevent blood clots, our registered nurses work with you to assess your labs and adjust your medication to ensure the right dose is maintained.
