North Oaks Relocates Physical Medicine & Rehabilitation Clinic to Enhance Care Coordination

Effective today, North Oaks Health System has relocated its Physical Medicine & Rehabilitation Clinic from Hammond Park Professional Plaza to North Oaks Rehabilitation Hospital, 1900 S. Morrison Blvd., Suite A, in Hammond.
The move enhances care coordination by placing the outpatient physical medicine and rehabilitation clinic within the rehabilitation hospital to create a more seamless continuum of care for patients.
The clinic will share space in Suite A of the Rehabilitation Hospital with North Oaks Occupational Health Services.
North Oaks Physical Medicine & Rehabilitation Clinic specializes in non-surgical treatment of conditions such as brain and spinal cord injuries, stroke, arthritis, carpal tunnel syndrome, and back and neck pain.
“By moving the Physical Medicine and Rehabilitation Clinic within the Rehabilitation Hospital, we are creating a more connected, patient-centered approach,” notes Michael Williams, Vice President of Orthopedic and Neurosciences Service Lines for North Oaks Health System.
North Oaks Physical Medicine & Rehabilitation Clinic patients will continue to be staffed by Physiatrist Thien Ninh, MD, and Nurse Practitioner Vyrl Traylor.
Hours of operation remain 8 am to 5 pm on weekdays.
