Nurse Practitioner Catherine Horn Joins North Oaks Endocrinology in Hammond

Nurse Practitioner Catherine Horn, APRN, AGNP-C,has joined North Oaks Endocrinology Clinic in Hammond. To schedule an appointment, call (985) 230-APPT [2778].
Horn specializes in adult gerontology primary care, focusing on managing and preventing chronic health problems. She is especially passionate about early intervention and education that gives patients the confidence to self-manage lifelong conditions like diabetes, improving their quality of life.
“What I enjoy most are the small wins like witnessing the transformative 'aha moments' when a patient finally understands their lab results and readings,” she shares. “There is nothing more rewarding than seeing a patient’s confidence grow as their A1c improves, their energy returns, and they realize they are in control of their health, rather than the disease controlling them.”
“And with those continued small wins comes long-term partnerships I get to build with patients, which I truly value,” she adds.
Horn will work closely with North Oaks endocrinologists A. Mannan Khan, MD, and Corey Majors, MD, who both specialize in diagnosing and treating conditions that affect the endocrine system. Common conditions treated at North Oaks Endocrinology include diabetes, thyroid disorders and thyroid cancer, osteoporosis, and metabolic syndrome.
Horn believes that, as an NP, her unique training equips her to help patients master the skills needed to manage their diabetes effectively. Her goal is to empower her patients through clear communication and support their treatment through personalized care plans.
“In diabetes management, success isn't just about a lab value; it's about understanding a patient’s life and providing the education they need to take control of their health,” she explains. “I find it incredibly rewarding to see the long-term impact of personalized care on a patient’s quality of life.”
Horn earned her bachelor's degree in general studies with minors in psychology, social work, and sociology from Louisiana State University in Baton Rouge, her associate’s degree in nursing from Our Lady of the Lake College in Baton Rouge, and her master’s degree in nursing from the University of South Alabama in Mobile.
She is certified as an adult gerontology nurse practitioner by the American Association of Nurse Practitioners (AANP).
Nurse practitioners are nationally certified, state-licensed medical professionals who can practice independently while working in close collaboration with their supervising physician.
North Oaks Endocrinology Clinic is located at 15795 Paul Vega, MD, Drive, Building 3, Suite 400, in Hammond. Clinic hours of operation are 8 am to 5 pm, Monday through Friday.
