Medical Necessity Form

If you require electricity for your medical care, such as the use of oxygen or refrigeration for your medication, each utility company has their own form that you will need to ask your provider office to complete. Completing the form does not guarantee that you will not lose power or will be the first to get power turned back on after an outage.

Please have a back-up plan. Completing the form can help the utility companies prioritize their work in the event of an outage (not a major disaster).
