Returning to the Community: Continuing your Recovery
Our goal is to return you to your daily activities as soon as possible so you can conquer, master and reclaim your life.
Before you return home, a customized follow-up program will be developed for you. Your case manager and our medical director will work with you to help ensure that your transition home is smooth and the services you may need are being provided.
LINK, a day rehabilitation program
When you no longer need to be hospitalized, you may transition to
LINK, a day rehabilitation program offered by North Oaks Rehabilitation Services. The
LINK program enables you to receive the same intensive, individualized therapy that you received while on the CMR Unit, then return to the comfort of your home and family in the evening.
The
LINK program bridges the gap between inpatient and outpatient rehabilitative care. To qualify you must participate 5 days per week and receive two or more of the following therapies:
Physical Therapy,
Occupational Therapy, or
Speech Therapy.
Upon discharge from CMR or LINK, we offer continued care through
outpatient rehabilitation, support groups and counseling services.
