During recovery, a patient will begin learning about the lifestyle changes and behavior modifications necessary for well-being.
Cardiac Rehabilitation – The Cardiac Rehabilitation program at North Oaks is nationally certified by the American Association of Cardiovascular and Pulmonary Rehabilitation and is only 1 of 13 programs in the state to hold this distinction. The program is offered in three phases:
Phase I – Inpatient care
Phase II – Monitored, supervised outpatient care
Phase III – Maintenance, outpatient care
Nutritional Services – Patients receive personalized instruction and education from dietitians on developing heart healthy eating habits.
Congestive Heart Failure (CHF) Program – North Oaks offers a comprehensive, educational program to help patients learn to manage CHF.
Anticoagulation Management Clinic (AMC) Program - The AMC offers point of care service for adult patients on warfarin therapy. Patients receive one-on-one assessment of labs and medication adjustment with a clinical nurse specialist following established guidelines.
