• Have you been a patient here before? If this is your first visit, who is your insurance carrier?
• What is the reason for your appointment?

If follow-up treatment is recommended for your illness or injury, our medical staff will ask you to return to your primary care physician.

Extreme Emergencies
North Oaks Walk-In Clinic is not equipped to treat life-threatening or possible loss-of-limb emergencies.

In the event of an extreme medical or potentially life-threatening situation, please call 911 or seek attention at the nearest hospital emergency department. If in doubt about North Oaks Walk-In Clinic’s ability to treat your condition, please call us at

Please be prepared to pay your co-pays and deductibles at the time of your visit. Cash, personal checks and major credit cards are accepted.

Please bring your insurance card with you so that we may make a copy of it and process your claim to your insurance company as a courtesy to you. If pre-certification is required by your insurance carrier for us to treat you, it is your responsibility to secure it.

Occupational Health Services
Getting employees back to optimal health with reduced time away from work is our priority. Our Sick Care program offers non-emergency care for client employees and their immediate family members. The Workers’ Compensation program focuses on immediate treatment, timely evaluation, appropriate referrals and rapid recovery.

North Oaks Walk-In Clinic is equipped with drug screening services that comply with federal collection standards, and we also offer preplacement physicals. For your convenience, 24-hour emergency worker injury coverage is available through North Oaks Medical Center’s Emergency Room, when North Oaks Walk-In Clinic is closed, or in cases of life-threatening injuries.
